Filing a Formal Complaint

The College of Denturists of Ontario has a formal complaints process. Each step is designed to ensure fairness, to both the person filing the complaint and the Denturist who is named. 

The Inquiries, Complaints and Reports Committee (ICRC) will investigate complaints in a thorough and objective way to determine if there is any evidence of professional misconduct, incompetence or incapacity.

Sometimes, you and a Denturist can work together to resolve any problems that arise between you about unsatisfactory care.  Before you make a formal complaint, the College encourages you to discuss your concerns directly with your Denturist.  

You must send your complaint in writing or a recordable medium such as audio or video recordings.

Please be prepared to provide:

  • as much detail as possible about your concerns
  • the full name of the Denturist you’re complaining about
  • the names and contact information of anyone who may have relevant information or may have been present when the incident occurred
  • your daytime and evening telephone numbers, e-mail address, and mailing address.

Step 1: Review the Complaints Process

Step 2: Complete the Complaint Form

Download the Complaint Submission Form and provide a detailed description of your complaint, either on the form or in a separate document or recordable medium.

Include the following:

  • Your full name
  • Your mailing address, telephone and e-mail contact information
  • The full name and address of the Denturist
  • A clear statement that you are submitting a complaint
  • Specific date(s) of the occurrence
  • Detailed description of the occurrence and your concerns
  • Type of complaint (denture problem, conduct problem, other)
  • Type of denture (full/partial/both, upper/lower/ both)
  • The names of other Denturists, health care practitioners or persons that may have relevant information
  • Discussions with the Denturist about the concerns and outcomes (if you have raised the issue)

Step 3: Attach any supporting evidence

Please forward all supporting evidence to the College - photographs, receipts, invoices, witness statements, etc.

Step 4: Mail, fax or e-mail the completed complaint form, description and evidence to:
